The GUIDE Model is a program launched on July 1, 2024, by the Centers for Medicare & Medicaid Services (CMS). The model provides a standard approach to care for families and their loved ones living with dementia, allowing them to live in their homes and participate in their communities for longer.
What Does the GUIDE Model Provide?
The main goal of the GUIDE model is to improve quality of life for both people with dementia and the family members that provide care for them through care coordination and management, caregiver education and support, respite care, and reimbursement for services.
Family caregivers receive specialized training and support services which include access to a 24/7 support line and community-based providers to supplement other needs. To address gaps in dementia care, CMS employs the following strategies: defining a standardized approach to dementia care delivery for model participants, providing an alternative payment methodology to model participants, addressing unpaid caregiver needs, respite services, and screening for Health-Related Social Needs (HRSNs). These strategies are crucial in providing a comprehensive approach to dementia care that has been missing in the realm of nationwide health care.
Personalized care management is essential to supporting family caregivers by providing respite services to give them breaks from caring for their loved ones and training to give them the best approaches to changes that may occur with someone living with dementia. The GUIDE model also ensures 24/7 support and connections to community-based organizations to address additional barriers to care.
The GUIDE model assigns a Care Navigator to caregivers and their loved one. This navigator helps coordinate and organize the care to allow family caregivers to provide the highest standard of care to their loved one through support from their interdisciplinary care team, and through a tailored care plan, allows people with dementia to work towards their goals and preferences.
Participants in the GUIDE program are Medicare Plan B enrolled organizations that also meet the requirements of the GUIDE program. For those organizations that cannot meet the GUIDE requirements, they can contract with other organizations, both Medicare-enrolled and non-Medicare enrolled providers to meet the requirements.
Examples of partner organizations include: Area Agencies on Aging, Tribal Elderly services, Caregiver Resource Centers, Alzheimer's Association, Adult Day Services, and Community-Based Adult Services Providers.
How can CAB contribute to GUIDE Partnerships?
Healthcare providers and organizations participating in GUIDE had two rollout dates. For organizations that met the requirements before GUIDE launched, the start date was July 1st, 2024. Organizations that did not yet meet the requirements have a delayed launch date of July 1st, 2025 to allow time for their programs and services to meet all of GUIDE's requirements.
GUIDE participants are Medicare Part B enrolled organizations and suppliers. These organizations that do not meet the requirements can partner with other organizations and suppliers to establish their dementia care programs and meet the requirements.
An interdisciplinary team is needed to provide the type of long term, in-depth care that GUIDE requires. These teams may involve caregivers, care coordinators or managers, doctors offices, and specialty care. For organizations without prior expertise in some of these areas, providing care under GUIDE's requirements may be more difficult.
